You also might benefit from checking that the output device is set correctly in the host.
What type of USB ports do you have? If it's 1.0 or 1.1 then that may go some way to explaining the problem. If it's 2.0 then that should be fine..

Everything seems fine in the host, It's USB 2.0. I've had this problem before, but it was fixed when I restarted the host, but it doesn't seem to be working this time
